Hospice services are available in a nursing home or other facility to provide an extra level of care from a team of experts in end of life care. Hospice of Jefferson County partners with your nursing home and works with them to provide the following services:
Hospice will work with the staff at the facility to provide a coordinated plan of care that reflects the Hospice mission; to provide expert, compassionate, comprehensive care and services to individuals and their loved ones experiencing life-limiting illness and loss.
Studies show that patients who are referred to Hospice Care as early as possible experience a far superior quality of life, with fewer hospitalizations and a decrease in symptoms. Referrals for Hospice Care can come from you, your loved ones, your physician, or your nursing home. Medicare-certified Hospices require physicians to certify that a patient's prognosis is expected to be six months or less.
Hospice care in nursing homes is covered by Medicare, Medicaid, and most insurance plans. Financial assistance may also be available. Please call 315.788.7323 for more information.
Our 12 bedroom Hospice Residence provides a serene and unique home-like setting for our patients and their families. The Residence acts as a home to our patients who cannot or choose not to stay in their own home or with a family member. Short-term respite care is also available for all Hospice Patients and can be arranged to give the caretaker relief for up to five days.
The Hospice Residence sits on five acres with wildlife, a pond and meditation garden. Our suites are spacious and include a sofa bed for family members to stay, private bathroom, refrigerator, television and wifi access. All 12 bedrooms have a covered patio that overlooks the pond and grounds.
The Residence also has a family room, large patio, walking trail around the pond, meditation room, and family shower facilities.
