Job description:
Hospice is seeking a full-time evening shift (3pm-11pm) LPN for our Hospice Residence. In the Hospice Residence, you will be part of an interdisciplinary team focused on symptom management and end-of-life care. A valid NY State LPN license is required. New grads are welcome to apply. Competitive pay based on work history. Shift Differential pay for evening and night shifts.
Paid time off, matching 401k and Health, Dental and Vision benefits are available to full time and part time employees. Rotating weekends with a safe patient ratio of 6 to 1.
Job Duties:
1. Provides nursing care according to the established plan of care and under the direction and supervision of the RN.
2. Provides emotional support to patient and family.
3. Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
4. Prepares light meals and/or snacks for patients as needed.
5. Performs light housekeeping in patient's immediate environment.
6. Adheres to New York State mandated reporting requirements.
7. Participates in Quality Assurance Improvement projects.
8. Works non-judgmentally with patients and families of many cultures, religious, socioeconomic backgrounds, and varied lifestyles.
9. Adheres to agency policies and accepted nursing practices and regulations.

NON-CLINICAL POSITIONS

Foundation Administrative Assistant
Reports To: Director of Foundation, Operations & Development
FLSA Status: Non-Exempt
(Full-time position: 40 hours)
Duties and Responsibilities: Provide administrative support to the programs, activities, and events under the sponsorship of the Foundation of Hospice of Jefferson County. Reporting to the Director of Foundation Operations & Development and will work with other leadership staff to ensure that the Foundation’s donor database and activities/events are organized and accurate to provide an excellent public experience.
Other administrative duties are as follows:
1. Maintains Hospice donor database (GiftWorks) to ensure accuracy, efficiency, integrity, and tracking of all donor communication/contact;
2. Responsible for entering and balancing weekly foundation deposits in GiftWorks and producing thank-you and acknowledgment letters;
3. Responsible for pulling Stripe reports, entering in Giftworks, balancing with deposits, and applying appropriate fees;
4. Balances foundation account and reconciles statement for Director of Finance and supplies information via Excel Pivot tables;
5. Runs monthly Hope Appeal, Memorial and Expansion reports for the Director Foundation, Operations & Development;
6. Produces and maintains GiftWorks’ Smartlists for all mailings and events;
7. Administrative support for Director of Foundation, Operations & Development and for scheduling for Hospice fundraiser and event planning meetings;
8. Assists with Hospice events from planning to post event review. Creates flyers for some Hospice and community-run events;
9. Facilitates and follows-up on solicited donations for auctions and prizes as directed;
10. Attends community events as assigned (health fairs, job fairs, community fundraisers, networking events);
11. Assists in taking photographs at events as directed and shares and preserves photographs as required;
12. Performs other related duties as assigned.
Qualifications:
- To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- The requirements below represent the required knowledge, skill, and/or ability.
- Excel intermediate/advanced Level preferred.
Education/Experience:
- Associate’s Degree required, Bachelor’s Degree preferred.
- Equivalent combination of educations and experience will be considered.
- Internship or one year’s experience in marketing, public relations, or accounting strongly preferred.
Knowledge, Skills, and Other Abilities:
- Efficient time management skills
- Professional behavior, dress, and attitude with the ability to work in partnership with all healthcare disciplines.
- Excellent verbal and written communication skills
- Excellent customer relationship knowledge and skills
- Ability to work well with all members of the Hospice staff, Volunteers, Board, and Community members
- Proficient problem-solving skills
Salary Range: $19.50 to $25.50
