Hospice is seeking a full and part time days LPN for our Hospice Residence. In the Hospice Residence, you will be part of an interdisciplinary team focused on symptom management and end-of-life care. A valid NY State LPN license is required. New grads are welcome to apply. Competitive pay based on work history. Shift Differential pay for evening and night shifts.

Paid time off, matching 401k and Health, Dental and Vision benefits are available to full time and part time employees. Rotating weekends with a safe patient ratio of 6 to 1.

Job Duties:

1. Provides nursing care according to the established plan of care and under the direction and supervision of the RN.

4. Prepares light meals and/or snacks for patients as needed.

Hospice of Jefferson County is seeking an immediate full time RN Case Manager interested in making a difference for those at the end of life. You will be part of an interdisciplinary team focused on palliative care and symptom management in the home or nursing home setting. 

A valid NY State RN License required. Competitive pay based on work experience and mileage reimbursement.

Job Duties include: The RN Case Manager will manage each patient’s care and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPNs) who will be directed / supervised by the RN. Overall responsibilities include planning, providing and coordinating quality end of life care.

Required Skills: Able to work independently, and complete documentation accurately and in a timely fashion. Valid driver’s license. Must be willing to travel throughout Jefferson County if required.

Hospice of Jefferson County is seeking an experienced Director of Patient Care to provide leadership, direction, and oversight of nursing services across our home care and hospice residence programs. This key leadership position ensures the delivery of high-quality, compassionate, patient- and family-centered care while supporting staff development, regulatory compliance, quality improvement, and organizational goals.

The Director of Patient Care provides clinical and administrative leadership to nursing and support staff, promotes continuity of care, and works collaboratively with the interdisciplinary team, physicians, patients, families, and community partners.

Key Responsibilities:

• Provide leadership, supervision, mentoring, and coaching to nursing and patient care staff.
• Oversee staffing and scheduling to ensure appropriate 24/7 patient care coverage.
• Lead recruitment, orientation, training, performance evaluation, and professional development of nursing staff.
• Ensure accurate, timely, and comprehensive patient assessments and individualized plans of care.
• Promote effective communication and collaboration among the interdisciplinary team, patients, families, physicians, and other providers.
• Respond to patient/family concerns, employee issues, complaints, and grievances and collaborate with the CEO on resolution.
• Oversee continuity and quality of care between home care and the hospice residence.
• Lead the organization's internal QAPI program and serve as Chair of the Internal QAPI Committee.
• Monitor compliance with New York State requirements, hospice standards, policies, and procedures.
• Participate in leadership, safety, emergency preparedness, and other organizational committees.
• Collaborate with the CEO, Finance Director, and Residence Manager in developing and monitoring the nursing department budget.
• Participate in policy development, performance improvement, staff education, and strategic planning.
• Maintain current knowledge of hospice regulations, healthcare practices, and emerging trends.
• Provide direct patient care when needed.

Qualifications:

• Current New York State Registered Professional Nurse (RN) license.
• BSN required; Master's degree preferred.
• Minimum 4 years of professional nursing experience in hospice, hospital, nursing home, or home care.
• Minimum 2 years of administrative/leadership experience.
• Strong clinical judgment, communication, organizational, leadership, and conflict-resolution skills.
• Demonstrated ability to lead teams, manage competing priorities, and promote a culture of compassionate, high-quality care.
• Valid driver's license.

Hospice of Jefferson County, Inc. is seeking a compassionate, professional, and dedicated Bereavement Coordinator & Hospice Social Worker to join our hospice team.

Position Summary

The Bereavement Coordinator & Hospice Social Worker provides compassionate psychosocial and bereavement support to hospice patients, families, caregivers, and members of the community. This position serves as a member of the interdisciplinary team and is responsible for coordinating and providing comprehensive bereavement services while also providing social work services to patients and families throughout the hospice experience.

Key Responsibilities

• Provide psychosocial assessments and social work services to hospice patients and families, with a focus on end-of-life, grief, and adjustment needs. 

• Collaborate with the interdisciplinary team to develop, implement, and monitor individualized plans of care. 

• Coordinate HJC's comprehensive bereavement program, including bereavement assessments, plans of care, follow-up services, correspondence, and closure. 

• Provide grief and bereavement support to families before and after a patient's death. 

• Coordinate bereavement support groups, educational programs, activities, and memorial services. 

• Develop and maintain bereavement resources, educational materials, and community programs. 

• Coordinate bereavement volunteers and staff assignments as appropriate. 

• Maintain timely, accurate, and comprehensive clinical and bereavement documentation. 

• Participate in interdisciplinary team meetings, staff meetings, quality improvement initiatives, and required education and training. 

• Promote hospice philosophy and end-of-life education within the community. 

• Maintain confidentiality and comply with HIPAA, New York State regulations, and mandated reporting requirements. 

• Perform other duties as assigned. 

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.  

• Two to four years of counseling, social work, human services, healthcare, or related experience preferred. 

• New York State Social Work license

• Hospice, palliative care, bereavement, or end-of-life experience preferred. 

• Valid driver's license and reliable transportation. 

• Excellent communication, interpersonal, organizational, and time-management skills. 

• Ability to work effectively and collaboratively with patients, families, physicians, healthcare professionals, volunteers, and community partners. 

• Strong computer and documentation skills. 

• Professional, compassionate, and respectful approach to working with individuals and families experiencing serious illness, death, and grief.
