CLINICAL POSITIONS

  • 1 LPN Part Time – Home Care (8am – 4:30pm)

    You will be part of an interdisciplinary team focused on symptom management and end-of-life care. A valid NY State LPN license and COVID vaccination are required. New grads are welcome to apply. Competitive pay based on work industry.

    $21 - $30 per hour based on experience. 


    Job Duties: 

    1. Provide nursing care according to the establised plan of care and under the direction and supervision of the RN. 
    2. Provides emotional support to patient and family.
    3. Maintains confidentiality, inclusice of HIPAA regulations, of all pertinent patient care information.
    4. Prepares light meals and/or snacks for patients, as needed.
    5. Performs light housekeeping in the patients immediate environment, as needed.
    6. Adheres to New York State-mandated reporting requirements.
    7. Participates in Quality Assurance Improvement projects. 
    8. Works non-judgementally with patients and families of many cultures, religious, socioeconomic background, and varied lifestyles.
    9. Adheres to agency policies and accepted nursing practies and regulations. 
  • 1 LPN Part Time (11pm – 7am)

    In the Hospice Residence, you will be part of an interdisciplinary team focused on symptom management and end-of-life care. A valid NY State LPN license and COVID vaccination are required. New grads are welcome to apply.  Competitive pay based on work history.

    $21-$30 per hour based on experience.

HOSPICE ADMINISTRATION POSITIONS

  • Cook, Hospice Residence

    Bring your Food Service skills to Hospice of Jefferson County. This position is part time with the potential to become full time. Responsible for adhering to policies and procedures, and federal/state requirements to ensure that food quality standards, inventory levels, food safety guidelines and customer service expectations are met. Responsible for maintaining records of food, supplies and equipment by performing the following duties.


    Salary Range - $17-18 hr.


    Duties and Responsibilities include the following. Other duties may be assigned.


    1. Serves food, and cleans utensils and dishes in residential kitchen.
    2. Follows standardized menus and recipes approved by a registered dietician.
    3. Maintains food costs within defined budget.
    4. Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
    5. Forecasts and plans the purchase of food, supplies and equipment.
    6. Maintains a daily accurate log of patients name, diet, special needs and requests.
    7. Responds to patient preferences to encourage participation in nutritional program.
    8. Maintains required records to include food production, inventory and meal counts.
    9. Ensures that established sanitation and safety standards are maintained.
    10. Oversees and participates in the preparation and serving of food.
    11. Interacts with patients, families other personnel and the public.
    12. Follows all department of health regulations.
    13. Responsible for miscellaneous grocery purchases to supplement bulk supplies.
    14. Performs other related duties as assigned.
